Case Management

When it comes to an individual’s health, social care and educational and employment needs, case management unfolds as a process whereby assessing, planning, implanting, coordinating and carrying out an evaluation is done in order to see the options and services required to meet this. This process also aims to create a cost-effective outcome, therefore using communication and available resources that are quality guaranteed.

Case Management for Children

Tehy Care has experienced case managers who come in handy when it comes to case management. They are a team of specialised individuals who seek to not only delve into the case with full energy but are known for their excellence in work and their depth of expertise.

A case manager needs to have skills that involve working with the whole family; this adds a bonus alongside having the right clinical expertise. This is because working with the whole family encompasses a network of family relationships which leads to establishing a successful wholesome outcome when it comes to providing focus and care for the child.

Working with Rehabilitation Providers

To provide great services, our case managers have not only polished the services they commission but also we at Tehy Care have concentrated on our partnerships with providers and strengthened them.

These relationships/partnerships have been built over the years this way we have gotten to know each other and build trust along the way. It has also enabled us to learn how they work in the effectiveness of their delivery, what they provide and monitor the outcomes as well.

Direct Employment

For clients who wish to employ care and support staff directly, our human resource professionals work with the client and family directly. This, of course, forms part of our robust approach in providing support to our clients and their needs.

We, however, understand that it can be a bit daunting and complex to choose care directly, that is why we provide complex care packages to our clients and families.
